Dental coverage was largely built around cleanings, fillings and extractions. Implants arrived later, and every carrier decided separately how to treat them. That is why one patient walks in with meaningful coverage and the next, paying a similar premium, has none for the same treatment.

What to Look For
Four Clauses That Decide the Answer
When we read a plan, these are the four things we look for first. They are also the four worth asking your carrier about directly, in these words.
Implant vs. Crown
An implant is a post, an abutment and a crown. Plans often file those under different categories, so one part can be covered while another is not.
The Missing Tooth Clause
Excludes replacing a tooth that was already gone before the plan began, sometimes years before. Ask whether it applies and whether it expires.
Waiting Periods
Many plans make you wait before major services are available. If you are close to the end of one, the timing of treatment matters.
The Annual Maximum
Every plan caps what it pays in a benefit year. Implant treatment often spans months anyway, so ask whether staging it across two benefit years helps.

Plans often classify those separately, so the crown falls under a category the plan pays toward while the surgical placement sits in a category it does not. Occasionally, when tooth loss came from an accident, an injury, or treatment for another medical condition rather than from decay. It is not the usual path and it depends entirely on the policy.
